TUBERCULOSIS SKIN TEST (TB Test)

A Tuberculosis Skin Test (TB) is required by the Department of Health for all employees working in a Health Care environment. A TB skin test is conducted to ensure health care providers have not been exposed to tuberculosis (TB) and aides in maintaining a physically healthy environment for students and employees.

Employees must complete and submit a negative TB test within the first 45 days of employment. Employees who fail to complete TB testing or who test positive for TB will not be eligible to continue employment at Gateway Academy.

Step 1: Schedule a TB Test

  • Contact The Salt Lake County Health Department immunization scheduling line at (385) 468 -7468.

Step 2: Attend Two Appointments

  • TB Test procedures include an initial appointment when the TB shot is administered.

  • There is an additional appointment within 48 hours to review the results of the TB test.

Step 3: TB Test Results

  • Turn in TB Test result documentation to Gateway's HR Department

NOTICE: As an employee of a school, you are required to maintain a personal record of immunizations, exemptions, or proof of immunity pursuant to Health Regulation #38, Certificate of School Employee Immunization Requirements. You are required to be prepared to produce these records to the Department of Health upon request, in the event of an outbreak or a vaccine preventable communicable or infectious disease. You may obtain exemption forms from the Department of Health.
